
Some of the points made in the first few sections on the topic of
breast augmentation are best illustrated with a little video segment. The
natural breast can only somewhat be mimicked by breast implants. Sometimes
the choices women make on goal size have an effect on how natural the outcome
appears. Similarly, the choice of implant placement Above
(as in Subglandular Breast Augmentation) or Below
(as in Submuscular Breast Augmentation) the pectoral muscles (particularly
when using saline-filled implants). Thin women such as the young
lady below (not my patient) usually have very little breast and muscle
tissue prior to surgery. The placement of even a moderate-sized implant
in this sort of case will lead to an un-natural appearance at least
for a few months even if the pectoral muscle is utilized to cover the implant.
If the implant is placed over the pectoral muscles, the fullness in
the upper part of the chest never really subsides. A very clear line is
apparent where the implant "starts" on the patient's chest. The implants
may seem "mounted" to the chest not moving with the patient's movement.
Personally, I am no a fan of this kind of result. I almost always seek to limit the size of the implant (within reason) and place it under the pectoral muscle to "soften" this upper chest "ledge" that is seen in the "Over The Muscle" placement. Take into account that for very thin patients, the use of the muscle can only somewhat soften the stark look of the implant below. Smaller implants tend to move better than really large ones as the chest tissues have room to allow them to bounce slightly when the woman walks or runs.
The Answer: Don't make the implant so large or accept a somewhat more foreign-looking breast.
The Compromise: Smaller implants don't result in larger bra cup sizes and don't make for as full a cleavage.

This woman at the right has obvious breast implants.
She is quite thin with breasts that project a good deal from her chest.
She also has a fair distance between her breasts. She is lovely but her
breasts seem a bit odd (my opinion).

See how the young ladies' breasts move little as she spins around. Also, notice their shape almost like half an orange. They seem to project too much.
There are two likely causes here:
A natural breast is soft. A natural breast bounces somewhat with movement. When proper restraint is used, often these characteristics can be obtained with breast augmentation. There are compromises here, so the patient's wishes within reason are considered.
